    Services:


    Diagnostic Assessment 

    A detailed report including biopsychosocial and diagnostic criteria with clinical assessment on the potential diagnosis and treatment recommendations. 


    Individual Therapy 

    Individual psychotherapy involving one on one sessions to create your goals, apply psychotherapeutic interventions and achieve results.


    Therapy Intensives

    A therapy intensive session is the powerhouse approach to therapy. It is goal-driven, longer in duration and occurs more frequently over a shorter period of time. It is best suited for those in a role where a mental health is interfering with their high level of functioning and are in need of rapid, long lasting resolution. Weekday and weekend scheduling.


    Accelerated Intensive Retreats

    The Accelerated Intensive Retreat (AIR) is a full day therapeutic retreat dedicated to you. Receiving full day of one on one care with a therapist focused on helping you achieve your desired goals for wellness. Experience the growth and wellness from your very own personalized intensive retreat.


    Corporate Intensive Therapy 

    Providing comprehensive mental health services to assess staff and engage leadership in protocols known to successfully identify, address, and resolve organizational barriers. Helping leaders and their team with work burnout, manage stress, and other work related issues. Available on site and on location. Inquire within to schedule.

  • Availability

    Typical hours of availability are 

    10:00am - 4:00pm Monday through Friday. 


    Appointment availability varies based on service and client need. Services may have evening and weekend scheduling availability, by request.

    Therapy is an investment into you.

  • Frequency of Therapy

    To achieve the desirable impact from individual therapy sessions may begin weekly and shift to bi-weekly or monthly. Therapy Intensives may be less.


    Accelerated Resolution Therapy suggests effectiveness in 1 - 5 sessions, but therapy length and needs can very based on each individual client. 


    Your investment of time and resources into your therapy will pay dividends throughout your life. 


    The frequency of your therapy will depend on many variables and can be discussed with your therapist.

  • Cancellations

    In the situation that you need to cancel or reschedule an appointment, notice must be provided at least 48 hours in advance, or you will be responsible for the full session fee.

  • Insurance

    EVO Counseling LLC is contracted  with Optum and Aetna insurance. 


    Insurance Information:

    Services may  be covered in full or in part by your insurance or employee benefit plan. Please call the number on the back of your insurance card and ask for member services. You will want to check your coverage carefully by asking the following questions:

    • Do I have mental health insurance benefits?
    • What are my out  of network benefits?
    • What is my deductible and has it been met?
    • How many sesisions per year does my insurance cover?
    • What is the coverage amount per therapy session?
    • Is approval required from my primary care physician?

    For Self-Pay Service:

    If paying out of pocket, please contact us directly. 


    Payment of services is completed on the day of service, and may be made by credit card (including HSA cards, when applicable).

  • Out Of Network (OON) Insurance

    Insurance may cover full or part of your individual therapy appointments. Insurances typically do not cover therapy intensives nor retreats. 


    We utiliize Thrizer for Out Of Network (OON) billing, allowing clients to have accessible therapy and not be limited by their insurance. 


    Thrizer works to submit your OON service to your insurance directly for reimbursement. They work with every insurance company that provides OON benefits!


    Contact your insurance provider to fully understand what reimbursement options may be available in advance. 


    Anyone who uses insurance or OON insurance reimbursement must be given a diagnosis, as insurance-covered treatment must be deemed "medically necessary."

  • Appointment Location

    Services are available in Wisconsin.

    In person appointments are held in the Quarry Arts building located at 715 Hill Street, in Madison, Wisconsin. 


    Tele-health appointments are  available anywhere in Wisconsin. Many of my clients prefer online therapy given tight schedules, distance, or a need for increased flexibility. Many clients also use the opportunity to work with me from elsewhere in the state, as it gives them greater choice in who they find the best fit with.


    The benefits of online therapy are many, and can include:

    • Reduced geographical limitations related to the therapist you can access or choose to work with.
    • Decreased commute and time spent in the car getting to and from your therapist
    • Increased flexibility for clients juggling the demands of particular jobs, childcare needs, etc.

  • Office Parking

    In person appointments are held in the Quarry Arts building located at 715 Hill Street, in Madison, Wisconsin. 


    Parking available:

    • Underground parking is available by pressing the red button on the keypad to enter the parking garage.
    • The neighboring Whole Foods grocery store has public parking available.
    • Street parking is available.

  • Good Faith Estimate

    Understanding Your Right to Receive a Good Faith Estimate of Expected Charges


    Under the No Surprises Act, health care providers and health care facilities are required to provide individuals who are not enrolled in any insurance plan and do not have coverage under a Federal health care program (that is, Medicare, Medicaid, or Tricare), or not planning on filing a claim with their plan or coverage, with an estimate of the cost for the services being requested.  This estimate is to be provided both orally and in writing.  It is to represent only an estimate, but one made to the best of the ability of your provider.


    While it does not have to be exact, the law gives you the authority to contest any bill that is $400 or more higher than the estimate.  If the provider has reason to believe that the estimate is inaccurate, the estimate can be updated but only for future services.  You can refuse to continue to seek care from the provider if an updated estimate of future charges is more than you wish to pay.  There is no requirement that once you start receiving services, you cannot stop.  


    If the billed charges are higher than the good faith estimate you can ask your provider or facility to update the bill to match the good faith estimate, ask to negotiate the bill, or ask if there is financial assistance available. 


    If you get a bill that is at least $400 more for any provider of this facility than the total expected charges for that provider or facility on the good faith estimate, there is a new patient-provider dispute resolution (PPDR) process available to you.  Under the PPDR process you may request a payment review and decision from an independent company certified by HHS.  These companies are referred to as Selected Dispute Resolution (SDR) entities.  The SDR entity will decide what amount you must pay if your bill is at least $400 more for any provider or facility than your good faith estimate from the provider or facility.


    The good faith estimate does not include services not provided by your provider that you may need, or your provider may recommend.
